Care coordination hub in line for digital award
Date: 11 April 2025A digital scheme designed to reduce hospital admissions and ambulance delays has been shortlisted for a Health Service Journal (HSJ) Digital Award.

The stack and unscheduled care coordination hub (UCCH) has been shortlisted for the 'Improving urgent and emergency care through digital' award.
The hub, which runs as part of a collaboration with Central London Community Healthcare NHS Trust, Essex Partnership University NHS Foundation Trust and Hertfordshire Community NHS Trust, provides a single point of access for ambulance clinicians and means lower acuity patients can be referred to a community setting, where appropriate.
Glenn Young, Deputy Director for Urgent and Emergency Care, said: “The hub makes a real difference to our patients and we’re incredibly proud that the work of everyone involved in the UCCH has been recognised.
"Working collaboratively with other providers helps us improve patient care and experience, as well as reducing pressure on other vital services in the NHS."
The HSJ Digital Awards shine a light on teams and organisations driving meaningful change through technology, improving patient outcomes, streamlining processes, and enhancing the overall quality of care.

The winners will be revealed at the HSJ Digital Awards ceremony on 26 June 2025.
